H3 centres on collective impact, with health, housing, homelessness, legal and settlement service providers working together in place to deliver and improve health, housing and homelessness services and options in the City of Wyndham.
The H3 Alliance mission, is to improve outcomes by advocating for and trialing, proactive and informed approaches to support people currently at risk of or experiencing homelessness across Wyndham.
The Alliance seeks to fulfil its vision and mission through a collective impact approach that:
- provides holistic, client-centred wrap-around services.
- Leverages and connects the multiple services offered by its partners.
- Develops and maintains a shared information system that responds to housing vulnerability for people in Wyndham;
- and provides ongoing supportive housing for people with complex needs who are experiencing chronic homelessness.
